Immunization Requirements
Defiance College also utilizes StarRez to collect Immunization Records.
All new students must submit a copy of their immunization records. NOTE: If you have a religious or other objection to any of the recommended vaccinations, please upload a letter stating your objections.
The following vaccines are recommended for all undergraduate students.
- MMR- 2 Doses
- Meningococcal- 2 Doses
- Hepatitis B- 3 Doses
- Covid-19
- Meningoccal B
- TDaP/TD
- Varicella
- Influenza
Instructions on how to provide documentation of immunization for Defiance College through StarRez is found below.
StarRez Immunization Upload Instructions
- Click on "DEFIANCE-Student SSO Login"
- The SSO Login is the same username and password you would use for Moodle, or myDC. It will prompt as a Micorsoft Sign-In.
- Click on "Immunization Upload" at the top of the toolbar.
- Review our recommended immunizations
- Upload a file or take a picture of the documentation, afterward click Save and Continue.
- Once on the Confirmation Page you have successfully uploaded your Immunization Record.

PLEASE DO NOT MAIL YOUR IMMUNIZATION RECORD TO THE COLLEGE. Once you have been assigned a Defiance College email address, visit www.medproctor.com to register your new account, using your Defiance email address. Follow their instructions for completing this process.
